Primary
Home Care
This is an attendant care services
program with services available based on income and resource
eligibility criteria. The services are known as:
Ø
Primary
Home Care (PHC)
Ø
Community
Attendant (CA)
Ø
Family
Care (FC)
The
difference among the programs is in the funding source and the
income and resource eligibility criteria. There is no age
limit to receive services for Primary Home Care or Community
Attendant services.
This
program is a non-technical, medically related, personal care
service. The service is for individuals with health problems
that cause them to be functionally limited in performing
activities of daily living. The service is available
statewide and does not have an interest list.
The three
types of Primary Home Care services available are:
Ø
Personal
care
– Help with activities related to the care of the client’s
physical health, such as bathing, dressing, grooming, caring
for hair and skin, helping with meal preparation, feeding,
exercising, helping with self-administered medication,
toileting, and transferring/ambulating.
Ø
Home
management
– Assistance with activities that support the client’s health
and safety, such as housekeeping, changing bed linens, doing
laundry, shopping, storing purchased items, and washing the
dishes.
Ø
Escort
– Accompanying clients on trips to obtain a medical diagnosis,
treatment, or both. This service does not include the direct
transportation of the client by the attendant.
All
clients, except those under age 21, have limits on the amount
of services they may receive. People under age 21 may exceed
program limits as long as their needs do not exceed their
Texas Index Level of Effort (TILE).
MINIMUM PROGRAM
REQUIREMENTS
To be
eligible for services, the following criteria must be met:
Ø
There is
no age limit for PHC and CA clients. Family Care applicants
must be at least 18 years old.
Ø
Be a
Medicaid recipient or meet Community Attendant services income
and resource limits.
Ø
Meet
functional needs criteria and need help with at least one
personal care task. Level of impairment is measured during a
functional assessment, which is conducted by a DADS case
manager during an interview with the applicant and/or
designated representative.
Ø
Have an
unmet need for services.
Ø
PHC and CA
clients must have a practitioner’s statement.
